What Is a High-Risk Pregnancy?

Pregnancy is a very special process. Every day of this period, planned as nine months, 40 weeks, or 280 days, is important and open to surprises.

In the past, obstetrics and gynecology textbooks described pregnancy as a normal physiological stage in a woman’s life and stated that it was most likely to progress in a healthy and uncomplicated way.

However, according to data collected since the early 1900s, when modern medicine began keeping more reliable statistics on human health, it has become clear that nearly half of all pregnancies involve one or more problems, or complications in medical terms. Pregnancy is therefore not entirely ordinary, but rather a very special period open to many risks.

Approximately 10% of pregnancies end in early miscarriage before the 10th week. Around 10% of ongoing pregnancies result in preterm birth, meaning that the fetus is born before it can breathe, feed, or survive independently and may require neonatal intensive care. In about 10% of pregnancies, the mother develops high blood pressure (preeclampsia and other pregnancy-related hypertension disorders), and another 10% develop gestational diabetes. If these complications are not managed properly, they may pose life-threatening risks for both mother and baby.

Around 1% of pregnancies involve chromosomal abnormalities or genetic defects in the fetus. Routine screening tests such as the first-trimester, triple, or quadruple tests may therefore show high-risk results and require further investigation.

Structural defects affecting fetal organs are detected in approximately 1% of fetuses. In such cases, it is necessary to investigate whether these defects are associated with genetic abnormalities and to provide detailed counseling regarding their prognosis before and after birth.

In 1–2% of pregnancies, the expectant mother already has an important systemic disease before pregnancy. During pregnancy, special care, medication changes, or dose adjustments may be required.

In about 1% of pregnancies, the fetus does not grow and develop as expected. In these cases, often related to placental insufficiency, the baby’s circulation must be evaluated with repeated Doppler ultrasound examinations, requiring close follow-up and carefully timed delivery.

Multiple pregnancies, especially those conceived through assisted reproductive technologies such as IVF, represent another important category of high-risk pregnancies. Monitoring begins with distinguishing identical from fraternal twins and, in identical twin pregnancies, frequent follow-up is required to determine whether twin-to-twin transfusion syndrome is developing and whether laser treatment is needed.

When all these factors are considered together, nearly half of all pregnancies can be classified as high-risk pregnancies.
